Athletic Forms & Policies
Students and parents are required to review and comply with the forms below in order for the student to participate in athletics.
The NCAA Guide for the College-Bound Student Athlete answers important questions for groups of people involved in the NCAA initial-eligibility process, including high school students who hope to compete in college sports an NCAA Division I or II school, parents, guardians, and family members of high school students, high school counselors and athletic administrators as well as high school and nonscholastic coaches.

Athletics Registration Form
Prior to any student participating in practices, inter-school practices, scrimmages, and/or contests, at any PIAA member school in any year, the student is required to complete a comprehensive initial pre-participation physical evaluation (CIPPE) and have the appropriate person(s) complete the first six sections of the CIPPE Form.

Central Catholic and UPMC Sports Medicine have established a contractual agreement to provide a Certified Athletic Trainer to aide in the prevention, recognition, evaluation, and treatment of athletic injuries.
